Patient Forms
To keep things simple, you’re welcome to complete your patient forms right at our office. Just arrive at least 15 minutes early and we’ll take care of the rest. We prefer that you fill out your forms online before your visit. We’ll text them to you ahead of time.

Not at all. Many of our patients don’t have dental insurance. We offer clear, upfront pricing and flexible payment options, and we’ll always review costs with you before any treatment begins.
What if I’m nervous?
You’re not alone — dental anxiety is very common, and there’s never any judgment here. Our team is known for gentle care: we’ll explain everything, go at your pace, and make sure you’re comfortable every step of the way.
How long is the first visit?
Plan for about an hour. That gives us time for your exam, any needed X-rays, and a relaxed, unhurried conversation about your smile goals.
Can I bring my kids?
Absolutely — we love seeing families! Children are welcome at both our Lombard and Aurora offices, and we’re happy to care for smiles of all ages.
